Registration

Once you have completed the admission application form, you will need to follow steps outlined below to officially register for the course.

1. Complete a Student Transaction Form (STF). (Complete sections A and C, provide an email address and sign the form.)
   
   
2. To complete Section C:
   

   Title:	Algebra in Secondary Schools
   Subject:	MAT
   Course Number:	504

   
   
3. Fax the STF to the Office of Enrollment Services – Juliet Black, Registration Department at (954) 262–2336.
   
   
4. Retain the STF for your records until you receive an email confirmation from a FSEHS Registration Representative.
   
   
5. A registration confirmation will be sent to your email account list on the STF.
   
   
6. Upon receipt of your registration confirmation, please request your textbooks.
