The Placement Reporting Form (Online): All students completing their field experiences in Miami-Dade County , Tampa or Orlando are required to complete the Placement reporting form (online) no later than the fifth week of class. Students completing their field experiences in Broward County, Palm Beach County or Jacksonville were placed through the NSU Office of Placement Services and should receive an email regarding what further appropriate steps that need to be taken in order to officially report their placement. Please refer to the field experiences handbook or website for details.

The letter of introduction : Complete the accompanying letter of introduction by filling in the name(s) and number(s) of the education courses for which you are registered during the entire sixteen weeks. On your first visit to the school, give the letter of introduction and your proof of security clearance to the school principal or assistant principle. For every visit to a school you will need to sign in and out at the front office in order to adhere to the school's safety and security policy.

The field experiences verification form: Complete the form by indicating the date and amount of time spent completing each assignment in your placement. At the end of the placement period, have the cooperating teacher complete his or her evaluation section and sign the form to verify that you have completed the observations. Submit the completed form to the course instructor. A copy of the completed form should be included in your program portfolio. Any students with documented severe deficiencies or concerns will be referred to the Office of Field Placements for facilitation of remediation and counseling.

All forms are available on the field experiences website:www.undergrad.nova.edu/EDU . You may obtain a copy of the clinical experiences handbook by downloading it from the clinical experiences website.

Successful completion of the Field Experiences

Students must successfully complete the assignments and submit the required documentation to their instructor in order to receive a passing grade for field experiences:

  • Field Experiences assignment for this course
  • Field Experiences Verification Form

NOTE: Field experiences are a required part of this course. Failure to complete the minimum ten hours of observation as specified in this appendix and the accompanying assignment may result in an incomplete grade for this course.

All teacher candidates, including non-education majors and certification students, are responsible for completing all field experiences procedures and will be required to utilize and/or submit the following field experiences documents:

  1. Field Experiences Checklist: All teacher candidates are required to complete the Field Experiences Checklist by indicating the date and amount of time spent completing each field experience assignment in each course. At the end of each course, the teacher candidate should have cooperating teachers sign the checklist to verify that they have completed the hours and assignments. Then, they should have their course instructors sign the checklist. Once all signatures have been obtained, teacher candidates should submit copies of the checklist to their course instructors for grading purposes.

  4. The Field Experiences Evaluation Form: Once teacher candidates have completed their field experience assignments, they should have their cooperating teacher complete the Field Experiences Evaluation Form. The teacher will evaluate them based on their field experiences for all courses taken during the semester. Teacher candidates should submit the completed field experiences evaluation form to their course instructor along with the completed field experiences checklist at the end of the course.

    The instructor will forward all forms to the Office of Placement Services. Once the field placement office verifies the information submitted, the coordinator of field experiences will assign a grade for the benchmark and field experiences for which the teacher candidate is registered.
