Clinical Experience

Submission of Documents

The request for placement form : All students completing their clinical experiences in Miami-Dade County , Tampa or Orlando are required to submit the completed Request for Placement Form to their academic advisor no later than the fifth week of class. Students completing their clinical experiences in Broward County , Palm Beach County or Jacksonville were placed through the NSU clinical placement office and are therefore not required to submit their placement documents. Please refer to the clinical experiences handbook or website for details.

The letter of introduction : Complete the accompanying letter of introduction by filling in the name(s) and number(s) of the education courses for which you are registered during the entire sixteen weeks. On your first visit to the school, give the letter of introduction to the school principal. For every visit to the school sign in and out at the front office in order to adhere to the school's safety and security policy.

The clinical experiences verification form: Complete the form by indicating the date and amount of time spent completing each assignment in your placement. At the end of the placement period, have the cooperating teacher complete his or her evaluation section and sign the form to verify that you have completed the observations. Submit the completed form to the course instructor. A copy of the completed form should be included in your program portfolio. Any students with documented severe deficiencies or concerns will be referred to the Office of Clinical Placements for facilitation of remediation and counseling.

Successful completion of the Clinical Experiences

Students must successfully complete the assignments and submit the required documentation to their instructor in order to receive a passing grade for clinical experiences:

  • Clinical Experiences assignment for this course
  • Clinical Experiences Verification Form

NOTE : Clinical experiences are a required part of this course. Failure to complete the minimum ten hours of observation as specified in this appendix and the accompanying assignment may result in an incomplete grade for this course.

All pre-service educators, including non-education majors and certification students, are responsible for completing all clinical experiences procedures and will be required to utilize and/or submit the following clinical experiences documents:

  1. The Request for Placement Form: All pre-service educators completing their clinical experiences in Miami-Dade County, Tampa or Orlando are required to submit the completed Request for placement Form to their academic advisor no later than the fifth week of the course. Pre-service educators completing their clinical experiences in Broward County, Palm Beach County or Jacksonville are placed through the NSU clinical placement office and are therefore not required to submit the proof of placement documents provided by the clinical placement office to their academic advisor.

  2. The Letter of Introduction: All pre-service educators are required to complete the Clinical Experiences Checklist by indicating the date and amount of time spent completing each clinical experience assignment in each course. At the end of each course, the pre-service educator should have cooperating teachers sign the checklist to verify that they have completed the hours and assignments. Then, they should have their course instructors sign the checklist. Once all signatures have been obtained, pre-service educators should submit copies of the checklist to their course instructors for grading purposes.

    Pre-service educators are required to sign in and out at the front office for every visit to the school to adhere to the school's safety and security policy.

  3. Clinical Experiences Checklist: All pre-service educators are required to complete the Clinical Experiences Checklist by indicating the date and amount of time spent completing each clinical experiences assignment in each course. At the end of each course, pre-service educators should have the cooperating teacher sign the checklist to verify that they have completed the hours and assignments. Then, have their course instructor sign the checklist. Once all signatures have been obtained, pre-service educators should make a copy of the checklist and submit it to their course instructor so that they can be graded on the course requirement.

  4. The Clinical Experiences Evaluation Form: Once pre-service educators have completed their clinical experience assignments, they should have their cooperating teacher complete the Clinical Experiences Evaluation Form. The teacher will evaluate them based on their clinical experiences for all courses taken during the semester. Pre-service educators should submit the completed clinical experiences evaluation form to their course instructor along with the completed clinical experiences checklist at the end of the course.

The instructor will forward all forms to the clinical placement office. Once the clinical placement office verifies the information submitted, the coordinator of clinical experiences will assign a grade for the benchmark and clinical experiences for which the pre-service educator is registered.
